Thank you for considering the Center for Sexual Health. In preparation for your visit, I am enclosing several forms. Please send us the completed information along with a copy of both sides of your insurance card. Prior to making your office appointment, we would like the opportunity to verify your insurance benefits so you are aware of the potential out-of-pocket expenses for your evaluation and treatment. Once we have received your information and pre-approved your initial consultation visit, we will contact you to schedule your appointment. This process may take 7-14 business days. This upfront administrative work will minimize paperwork at your visit and enable us to make efficient use of your time at our consultation visit. We appreciate your patience.
